⚽ PLAYER WAIVER & ASSUMPTION OF RISK

Effective Date: 30 August 2025
Applies to: All participants in 5s LIVE leagues and events

By registering or participating in any 5s LIVE event, you (“the Player”) acknowledge and agree to the following:

1. Voluntary Participation – You voluntarily choose to take part in 5s LIVE matches and understand that football is a physical activity that carries inherent risks of injury.

2. Assumption of Risk – You accept all risks associated with participation, including but not limited to collisions, slips, falls, or contact injuries.

3. Fitness to Play – You confirm that you are physically fit and have no known medical condition that could make participation unsafe.

4. Medical Disclosure – You must inform your Team Captain or 5s LIVE staff of any relevant medical conditions prior to play.

5. First Aid & Emergency Response – 5s LIVE staff or venue personnel may administer basic first aid in the event of injury and, if necessary, contact emergency services.

6. Insurance – 5s LIVE provides public liability and venue insurance. Personal injury insurance is the responsibility of each player.

7. Liability Waiver – You release 5s LIVE Ltd, its staff, and affiliates from any claims for injuries, losses, or damages sustained while participating, except in cases of proven negligence.

8. Media Consent – You consent to being filmed and photographed during matches, and grant 5s LIVE full rights to use such media for promotional, commercial, or social media purposes.

9. Acknowledgement – By playing, you confirm that you have read, understood, and accepted this waiver.
